I,
as the parent or legal guardian of
(hereinafter referred to as “my child”), hereby acknowledge that my child is voluntarily participating in training sessions, classes, programs, and activities (hereinafter referred to as “activities”) offered by Train 106 LLC Sports Training Company (hereinafter referred to as the “Company”). I understand that the activities may involve strenuous physical exertion, including but not limited to running, jumping, lifting weights, and other activities that may be hazardous.

I recognize that the activities may involve risks of injury, including but not limited to cuts, bruises, sprains, fractures, and even more serious injuries such as paralysis or death. I am aware of the risks and hazards inherent in my child's participation in the activities and I voluntarily assume all such risks on behalf of my child.
I agree to ensure that my child follows all rules and instructions given by the Company’s instructors and staff members. I understand that any failure to do so may result in my child being denied further participation in the activities.

In consideration of my child being allowed to participate in the activities, I hereby release, waive, discharge, and covenant not to sue Train 106 LLC, its officers, directors, employees, agents, and volunteers (hereinafter collectively referred to as “Releasees”) from all liability to me, my child, my child's personal representatives, heirs, and next of kin for any and all loss or damage, and any claim or demands therefore on account of injury to my child's person or property or resulting in my child's death, whether caused by the negligence of the Releasees or otherwise, while my child is participating in the activities.

I further agree to indemnify and hold harmless the Releasees from any and all claims or demands made by any other party arising out of my child's participation in the activities.

I have read this Liability Waiver and Release and fully understand its terms. I understand that I have given up substantial rights by signing it on behalf of my child, and I sign it freely and voluntarily without any inducement.
